Most gem grinds in Diablo 4 feel like background noise until you chase a Flawless Horadric Gem. Then the numbers get silly. With the right Escalation loop, though, Diablo 4 Items aren't the only shortcut; smart routing can turn a miserable evening into a focused farm.
Why the Third Floor Is the Real Prize
Escalation runs aren't about clearing every room once you have the loop live. Clear floors one and two normally, then slow down at the second portal choice. With Branching Pathways unlocked, you get three options and can steer the third floor toward a specific reserve plus Dark Omen. That pairing is the whole trick. You loot the third floor, leave before completion, and come back to refreshed chests and another ambush. Miss either affix and the run is still playable, just nowhere near as juicy.
Make Every Reset Count
Once that pair appears, stop treating the floor like a standard dungeon. Open every Horadric chest first, because targeted reserves scale far better than the vague Gem Reserve roll. At Torment 12, a good chest can throw out roughly 270,000 fragments for the gem you actually need. Dark Omen does the heavy lifting: its stalking boss can drop about 750,000 each time. Kill it, grab everything, then leave the end boss alive. The exit portal is bait. Taking it finishes the Escalation and ruins the repeatable third-floor state.
